Liability Waiver

In this agreement, “Camp Organizers” means Kervin Kenton, Kenton Elite Sports LLC, UCAM CF, and their coaches, staff, volunteers, agents, and venue partners. Kenton Elite Sports LLC is the company under which Kervin Kenton and the rest of the camp team are protected. “Participant” means the player named in the booking. “I” means the participant if 18 or older, or the parent or legal guardian who completes the booking for a participant under 18.

I understand that football (soccer) camps, tryouts, games, and fitness testing involve physical contact and strenuous activity. Known and unknown risks include, but are not limited to, sprains, fractures, concussions, heat-related illness, cardiac events, collisions with players, equipment, or goalposts, uneven or wet playing surfaces, weather, and travel to and from the venue. Serious injury, permanent disability, or death can result.

I voluntarily accept all of these risks on behalf of the participant. I confirm the participant is in good physical health, has no condition that makes participation unsafe, and has been cleared by a physician if one was required. I agree that the participant will follow all coach and staff instructions and venue rules, and that the Camp Organizers may remove the participant from activity for safety or conduct reasons without refund.

To the fullest extent permitted by law, I release, waive, and agree not to sue the Camp Organizers for any injury, illness, loss, or damage to person or property arising from participation, including claims based on the ordinary negligence of the Camp Organizers. This release does not apply to gross negligence or willful misconduct. I agree to indemnify the Camp Organizers against claims brought by or on behalf of the participant that are covered by this release.

The Camp Organizers are not responsible for lost, stolen, or damaged personal belongings brought to any camp location.

Medical Authorization

I authorize the Camp Organizers to obtain and consent to first aid, emergency medical treatment, transport by ambulance, and hospital care for the participant if, in their reasonable judgment, it is needed and I cannot be reached right away.

I understand that camp staff are not medical professionals unless stated otherwise, and that first aid on site is limited. I agree to pay all costs of any medical care, treatment, or transport the participant receives, and I accept that my own or the participant’s health insurance is the primary coverage.

I agree to share with camp staff, before the camp starts, any allergy, medication, injury, or medical condition that could affect participation, and to send any required medication (for example an inhaler or epinephrine auto-injector) with the participant. Health information shared with the Camp Organizers is used only for the participant’s safety.

Photo/Media Release

I grant the Camp Organizers permission to photograph, film, and record the participant during camp activities, and to use the participant’s name, image, likeness, voice, and performance statistics in websites, social media, recruiting materials, scouting reports, and promotional content, in any media, without payment.

These materials remain the property of the Camp Organizers. The participant’s home address, phone number, email, and date of birth will never be published.

I may opt out of promotional use at any time by emailing contact@kervinkenton.com. Opting out applies to future content and does not require removal of materials already published or printed. Footage used privately for player evaluation and scouting is not affected by an opt-out.

Camp Terms

A spot is held only after payment is completed. Camp dates, times, and locations may change because of weather, venue availability, or safety concerns; if a camp is cancelled by the Camp Organizers, the camp fee is refunded or moved to another date.

Attending a camp or tryout does not guarantee selection, a roster place, a scholarship, or admission to UCAM or any other club or university.

I confirm the information I submit in the booking form is accurate. This agreement is effective from the moment the booking is submitted, applies to every session of the booked camp, and if any part is found unenforceable, the rest remains in effect.
